Hi Jason. Unfortunately I have encountered something very similar. I had an ich outbreak in one of my tanks, and along with the ich, my siamese algae eater's snout starteed to deteriorate. There were also smaller lesions on its sides. It looked like something had tried to bite its face off. Its flesh continued to melt away after the ich was cured. I never foun a cure and it eventually died. Luckily none of the other fishes aquired the disease. Sorry I can't give you any advice except quarantine that fish ASAP. Good luck. ---Don
