It's always possible your skinny fellow has a low-grade case of Hexamita. I'm not aware of any adverse reactions in loaches to metronizadole and that seems to work well on hex (at least it has in my tanks). Hexamita is a real problem but it seems to be pretty easy to get rid of...

You might watch during feeding-time and make sure he's getting food, he may not be competing at feeding time and ending up with scraps or nothing.
