worms, worms, worms, worms.......

About 3 weeks ago the water in my tank started to be unusualy cloudy with a green tint. I did a half water change 2 weeks ago and it cleared a bit so this weekend Ive changed half the water again and the water has cleared almost. The problem is that about 15 minutes after the water change Ive looked into the water and there are worms every where in it. The worms are about 5mm long very thin and white. They settle onto the gravel alright but my fish are now eating them. My filter is getting rid of some of them but not all. I think they might have come out of the gravel when I poured the water in and disturbed it. Iam woried what they might be and if they are
