The problem is simply your tank is dirty! You need to change more water more frequently. You need to siphon your gravel weekly. You need to rinse your filter media weekly. Your nitrates are grossly high. They should be under 20 ppm. I'd change 30% of your water, take a nitrate and p.h. reading, and then in a couple days or even maybe the next day, change another 30% of your water. Make sure you siphon your gravel. I've been there, and I know what the mistakes are. Thankfully a very knowledgable person on here advised me to what I was doing wrong, and I've maintained low nitrate readings and stable p.h. readings ever since. Your tank p.h. should not differ from your tap p.h. And, for the hell of it, get a tapwater nitrate reading too. Don't add chemicals...it's not going to work. Change the water. Water changes should be your answer. Give it a couple weeks, and you should see a positive change in your tank. I've also been told by the same person who advised on water changes, that loaches are extra sensitive to high nitrate readings. So get those nitrates down fast. The only way is through water changes. Let me know though what your tap nitrate reading is. Finally, when you get yourself organized, you should go live plants. The plants have made an INCREDIBLE difference in my nitrate readings. I've already got 20 ppm of nitrate coming out my tap, so the plants were a necessity for me.
