I'm just guessing, but it sounds like the swim bladder to me. I've seen this as the last stage of internal bacterial infections in various tetras. I had a head-and-tail-light tetra die recently with similar symptoms, along with other bacterial-type symtoms like bloating...but no other fish in the tank caught the disease. There were no loaches in the tank in question, but there were other tetras and discus - all still OK (crossing fingers) since this happened.

I can't recall - did you have tetras in the tank? Tetras do tend to car
