Kuhli Loach: periodic stress &/or death of unknown etiology . . .

I love kuhli loaches and would like to keep them in my aquarium, but appear to be having periodic occurrences where they become stressed and/or die.

Periodically, they seem distressed and come to the surface of the aquarium. Distress seemingly expressed by them alone; not by the plecostimus or any other aquatic animals. A water change seems to make this distress disappear.

Then every year or so, they just die. They show the distress I described above and do not respond to the water change. I then replace them and the new ones die quickly. And meanwhile, all the other animals I have in this five gallon aquarium (plecostimus, white clouds, betta,
