New clowns and carbon?

Recently got some cloens and one of them has a white spot on his "eye brow" this seems to be spreading and the other one has black splodges within its bands when he greys out, what I want to know is Iam thinking about getting the owner to medicate them before things get worse (its not my tank) and I was wondering what to do with the carbon as Ive never had to deal with it? Do I take it out and then medicate and how do I keep the bacteria alive? Any help would be welcome as my friend is new to fish keeping and Iam new to carbon :)
