Sick (?) clown

Please help!

Yesterday I bought 3 Clown Loaches (all about 2.5-3") and placed them in a cycled quarantine tank.

Today, I woke up to find one of the loaches missing... after looking around, I found that he had jumped through a 1" hole in the cover. Needless to say, he was barely breathing and didn't survive.

Now, I am concerned about one of the other loaches. This one was very shy when I put him in the tank and doesn't come out. But, this morning (when I was looking for the other loach) I noticed that it is swimming strangely (a little bit on its side) and breathing rapidly.

The tank was just cleaned and about 50% of the water was replaced before adding the Loaches yesterday. I treated the water, so it shouldn't be chlorine. Ammonia is in the safe range and the pH is about 6.8, so I don't know what it co
