Sick Clown Loach

My clown loach has always been shy, hiding in a ceramic log I have in the aquarium. Lately, he is coming to the surface of the tank and hanging there vertically. When he swims he is not as graceful, sometimes tilting to one side, then other times he swims find and looks for food. I have watched him eat a few fish food flakes, and it seems he has trouble swallowing them.

Any ideas? He is not in the same tank as the dojo loaches, he is in with gold gouramis, serpae tetras, black skirt tetras, 2 plecostomus, a cory, 2 kuhlie loaches, a swordtail and an african butterfly fish.
