Concaved-head disease in clown loaches ?!?!

Hello guys, haven't come here for a while, been busy with my practicum/rotation thing.

My battle with my clown's disease was not very succesful. Sorry because this is going to be a long post. Let me recap what happened.

My "new" (3rd) small clown started with some weird concaved line along each side of the head, with no signs of wasting in other parts of the body. He's then been gradually decreasing in strength and finally died without a trace in my tank while I was away.

After I was back I did some major water changes and maintenance. All other fishes including the two remaining original clown loaches were fine. Then all of a sudden one of them died within 2 days from an unknown disease. When I first noticed it he was already gasping and his body was covered with slime/fungus looking things.

So, having only one clown left, who is still very healthy and energetic, I decided to get him company. I bought two new clown loaches. They have been doing fine and gaining weight.

Now comes to my most recent problem. My big(er) original clown is sick! He, all of a sudden caught that slimy fungus looking thing and become weak. It might have been velvet but I'm not sure, looked like a little bit everything. This time I acted quick, and I used maracide and Aquarium Pharmaceuticals' fungus cure (some general antifungal + antibiotic). It's been about 3 days, the outside symptoms have disappeared and his skin is looking somewhat healthy again. But he's now hiding all the time and wouldn't eat. I caught a glimpse of him and he seem to be develping that concave in the head!!... back to ground 0???? (p.s. This whole time the 2 new loaches are healthy and kicking)

What should I do?? Does this ring a bell in any of you? I really don't wanna lose him cuz he's the only one left from my original pair that I got since they were babies and I was a newbie... they've been through lotsa shxts and survived but now seem to be sucumbing to this weird unknown disease!
