Challenge to diagnose-PIC *PIC*

OK, you guys please view these pics and help me diagnose the disease. Look carefully at the top of the fish ahead of the dorsal. It's washed out, grey, with yellowy patches. Second pic, just shows the fin rot, I think secondary infection, I have Maroxy in for that. Also, the Maroxy instructions say to "use as needed, not more than 5 days". I take that to mean repeat daily, so far I've put in three treatments,also I raised the temp to 78 F and am leaving the light off, in case it's velvet. I'm probly gonna try Maracyn-Plus, if I can find it, not sure what to do about the plants.- TX
