Disease Identification?

My school of Rummy-Nose have some sort of disorder. I've lost two so far, and about to have another go belly-up.

They get what looks like large, flat spots on them. (The first had 3 of these, the next, only one). These "spots" look somewhat like a large, flat, scale that's gone brilliant white. Hours later, the fish seem to lose their equalibrium, then die within 48 hours after the spots appear. No gasping, or other signs. I'm clueless as to what this might be. Anyone have any experience with this? Treatment? This is in a tank with a good number of plants.
