Help.. It's Happening again

About a month ago I lost 2 of my 7" clown loaches with no indication of what was happening. I just woke up one moring and they were dead, after being normal, healthy fish. Nothing else affected. I have just noticed one of my other 5" loaches going the same way, but he's hanging in there. Once again, ph is fine, no nitrates, nitrites, etc...etc. Last time i thought it could have been a water change i had done 3 days before, i.e increased chrlorine used for dosing the pipes. But it's about 10 days since I did my last water change and he's been just fine till now. I have had loaches for over5 years, and I'm totally confused. Here are some photos of the poor bugger. Can anyone help before
