brown spots on loach

I really have two questions. First, my clown loach has brown spots. I've had her for 5 years now and she's always been pretty healthy. Just the past week she's gotten the spots. They are not fuzzy like a fungus. They are flat and brown/grey in color. Secondly, I had two other much smaller clown loaches. I had them for less than a year and they are gone. They've simply vanished. I've looked under my driftwood, big rocks, behind plants... even on the floor. Could they have been eaten? Their tank mates were the following: angel fish, my big clown loach, pleco, ottos, an algae eater, and some mountain minnows. Any of them sound like the culprits? Thanks.
