OT: Cloudy Tank

I have a 25 gallon planted aqaurium. No matter how many water changes i do it always returns to its cloudy state. It has 5 medium sized angel fish that have been in their several monthes, and one pleco. It has a DIY CO2 thingy, a fluval 104 canister filter. The plants are 6 small amazon swords and two very large 24 inch tall amazon swords in the corner. The substrait is an inch of flourite with 3 inches of fine sand i bought and rinsed very well from Home Depot. The lighting is a 65 watt. The plants and fish are fine but after my weekly 50% water changes back to a sort of greenish hue in a day or so. I can only see about half way into the tank. Whats causing this and what can i do to fix this?
