Loaches are dying... help?

I have a 100 gallon tank with all kinds of loaches: clowns, blue redfins, botia longiventralis, striatas, skunks, tigers, ornate tiger sands, yoyo, and a kuhli. One four-striper clown I had for a year was in shining health a week ago. In two days he died, even with medication. In the past couple months, 7 clowns have died, yet my 8 year old and 4-5 year old are fine and happy. One tiger loach I've had forever, he's fine, but the new little guy is wasting away while the ones at our fish store are fat and healthy. I checked pH, hardness, buffering capacity, all is fine. The water is changed about once a week. What is going on?

Beyond that, every single live plant I put i
