clowns dieing like flies

HELP...
I have a new 12 gal f/w tank that I believe has already cycled(over 1 month old). my ammonia &
nitrite levels are very high.I've been doing partial water changes every day(2gals a day/with
measured conditioner & 1/2 table spoon of aquarium salt/for 3 days/6 gals total).(same temp)
I only feed a little once a day.
My ammonia/nitrite levels are not going down
& every day another clown loach dies shortly
after the water change!all other fish are thriving.(betta/gourami/shark/mollie 5 total)
I think I'm better off ignoring the levels!
ANY OPINION IS WELCOME!!!!!!!!!!
